Nov 4, 2025 · You can file an appeal if Medicare makes a coverage decision that you think is a mistake. If your appeal is successful, Medicare will reverse its coverage decision and pay for eligible services.

If the hearing request is unsatisfactory, a beneficiary may request a review from the Medicare Appeals Council (MAC). The request must be made within 60 days of receipt of the hearing decision.
